What is SAIL?

SAIL is a service designed to help you live safely and independently at home. We work with you to identify areas where you may need help and connect you with local services.

How does SAIL plus work?

We start off by having an informal chat with you over the telephone to see how we might be able to help. You can see the SAIL Plus journey below.

Sometimes we can offer you the help you need straight away. Where we can't help we'll put you in touch with the right person/service. Or if you think you need a bit more help we can start the Sail Plus process.
We can meet you in your home or anywhere you feel safe
We use something called the Rickter Scale to assess where you might need help. Everything we do puts your individual goals at the centre.
A support plan is created together and additional referrals are made. We jump to action, connection you with social care, befriending, Handifix, MYsocial and pendant alarm

SAIL Plus is for carers, people with disabilities or anyone over 55.

How do I refer myself to SAIL?
