SAILplus is for those who have complex needs, a range of things going on, and require a bit more than information or support over the phone.
SAILplus clients could require a mix of social prescribing, care navigation, brief intervention, information and advice.
The SAILplus team will make initial contact over the phone to discuss the areas of support listed on the referral and discuss any further support required.
If the client requires a home visit/ meeting or guided conversation the SAILplus worker will arrange this. The SAILplus workers can meet clients in their home, in the community or anywhere they feel safe and comfortable.
At the heart of these meetings are SAILplus conversations. The SAILplus conversation uses the Rickter Board scaling tool enabling the conversation to be client-focused, wide-ranging and allows the client to explore ‘what matters to me’.
The client can establish a current state (baseline), explore a preferred future and together create a support plan. These scores are recorded on the client’s Charitylog record. Additional referrals are made where necessary and appropriate.
SAILplus are able to support a client for up to three months. Additional meetings and conversations may also take place to support the client further.
When closing a case the SAILplus team will revisit the client to follow up and measure change. They will complete a final Rickter conversation. The scores allow us to see how things have changed overall.
